The future of digital health is already here — and it’s happening in Estonia!

Estonia will be the guest country at HIS 2025 — a choice that reflects its global leadership in healthcare innovation. Known as the most digital nation in the world, Estonia has transformed its healthcare system into a 100% digital, interoperable, and citizen-centered model.

Despite a population of only 1.3 million, the country demonstrates that well-implemented digital solutions are scalable and applicable to larger realities, such as Brazil. Tallinn, its capital, was recognized by the UN as the city with the best digital services in the world. Estonia proves that the difference doesn’t lie in size, but in the decision to innovate.

ESTONIA IN NUMBERS

  • Annual contracts with EHIF based on service packages;
  • Mixed payment system: case-based (DRG), fee-for-service, and global budgets.

Most major hospitals are public, with services also provided by private contractors;

 

  • 20 public hospitals included in the Hospital Network Development Plan (HNDP);

DIGITAL ECOSYSTEM

CLINICAL DECISION SUPPORT SYSTEM

  • EHIF’s system assists physicians in making clinical decisions;
  • Integrated with the national health information system;
  • Provides real-time alerts and recommendations;
  • Helps avoid drug interactions and identify risks;
  • Improves care quality and reduces medical errors.

RESPONSIBLE ORGANIZATION

  • TEHIK (Health and Welfare Information Systems Center):
  • Responsible for developing and maintaining Estonia’s digital health infrastructure;
  • Manages the national health information system and ensures interoperability;
  • Develops digital tools to improve system efficiency;
  • Ensures security and protection of citizens’ health data.

CURRENT HEALTH INNOVATIONS IN ESTONIA

Since 2000, the Ministry of Health has maintained a national BioBank with biological samples and health data from over 200,000 Estonians (around 20% of the adult population);

Used for genetic research and development of personalized medicine;

Integrated into the national health system to correlate genomic and medical records;

Now applied to AI-based early cancer screening, aiming to make personalized medicine a reality for all citizens.

Due to financial constraints, AI is essential to maintain the efficiency of Estonia’s health system;

€10 million is invested annually in health-related AI development;

Current AI applications:

  • Automation of medical documentation;
  • Detection of medical billing fraud;
  • Clinical decision support.

Tanel Tera is Head of the e-Services Management Department at the Health and Welfare Information Systems Center in Tallinn, Estonia, since September 2019. He leads the development of digital services in healthcare, social security, data, and labor, managing a team of 130 specialists. With a background in IT Systems Management from the Kehtna School of Economics and Technology, Tanel is a seasoned leader in the digital transformation of Estonia’s health and welfare sectors.
